The moving water was a nightmare from a blending standpoint.  The photomerge in CS5 was otherwise easy, albeit time consuming.

I've taken very large panos a few times with my motorized head.  The resulting shots are massive in size.  When scaled down, they are very sharp.  I haven't done a side-by-side comparison of my large panos against, say, an equivalent FOV shot with my TS-E 24.

The 60D has a tendency to overexpose from 2/3 stop to 1 1/3 stops, although it is usually 2/3 a stop.  Use exposure compensation and drop it down 2/3 of a stop. 

In aperture priority mode, take a test shot, look at the histogram, and look for blinkies.  Adjust the exposure compensation up or down based on the distribution of the histogram.  If you need to limit the light to the lens to shoot wide open, as others have noted, you should probably use an ND filter.

It sounds like you need lighting kit(s) for the macro, indoor, and outdoor work.  I shoot natural light about 99.9% of the time, so I can't advise you on appropriate gear.

I have not used the pinhole cap, but I have used the Holga lens for Canon.

It looks like a lens, but it is only a pinhole.  I not noticed dust issues with it, but I do not shoot with it often.  Although it will mount on both crop and full-frame cameras, the vignetting on full-frame is too extreme.  It is best to only shoot a pinhole with a crop camera.

Although the shots are in color, I generally prefer to convert to black and white.  It's a liberating way to shoot because you do not rely on sharpness or technology.  Composition is the critical factor.

I have the old style of the bag, which has two non-padded pockets on the outside.  The new style only has one pocket.  I can fit the 24-105 or 24/1.4II in the pocket of mine, with my 5DII (with RRS L-bracket) and 70-200/2.8II in the main compartment.  It is comfortable to use.  You can add a padded lens case on the side, too.

Any (expensive) ball head from Arca Swiss, Markins, or Really Right Stuff will do what you ask. The question then becomes the head's usability. How smooth does it operate, how much does it weigh, do you like the layout of the controls. I will offer another vote for Markins. The Arca Swiss monoball and the BH-55 are as solid as a mountain, but they are large and heavy....The only negative I can say about the Markins, is that they puzzlingly placed the spirit level on the clamp as opposed to beside it...
+1.  I haven't pushed down on my BH-55 or my Markins Q3T, but both lock down tight with no droop, and both are super stable.  I prefer the tension adjustment on my Markins over the RRS (the RRS either feels too stiff or too loose).
