Does somebody know where I can order a rear mount for a 1.4x III extender?

In what country?

You can get one from Canon, but its a bad idea. They must be shimmed or selected to match the extender, just a 1/10,000 inch out of true will affect the image.

Send it to Canon to have it replaced if its damaged.
