Would this concept work with a Macro lens.
A tilt / shift Macro lens could have a great depth of field.
Rather than having a dedicated tilt shift macro lens you'd have a macro lens and an adapter.

Yes.
These kind of EF to EF adaptors already exist, they turn every lens into a macro lens:

I don't see why the canon macro lenses wouldn't work. They should retain infinity focus as the adaptor takes up the space between the sensor and the lens where the mirror box would usually be.
